Business Card and Name Badge Order Form

  • Please use the form below to provide the Office of Communications with information for your business cards and name badge. The Office of Communications must also receive approval of your Purchase Request Form (PRF) from Finance prior to ordering business cards or name badges. You will need the following information for your PRF: name badge with magnetic fastener, purchased through Ryder Engraving,1029 Hazelton Etna Rd. SW, Pataskala, OH 43062, cost $7, and 1 box of 500 business cards, purchased through Chapman Printing, PO Box 2867, Huntington, WV 25728, cost $67.
    Allow 2-3 weeks after approval for printing and delivery. If you have questions, call the Office of Communications at (304) 929-6727. This form will ONLY accept email addresses. If you input a different email address, it will cause an error and your form will not submit.

  • Business Card Information

  • You will receive a PDF proof of your business card via e-mail. Please review it carefully and either send approval or changes to the Office of Communications. Cards cannot be printed without approval.
  • Badge Information

  • As short as possible; abbreviate if necessary.
  • Accepted file types: pdf, Max. file size: 256 MB.
    Please upload your approved Purchase Request Form (PRF) as a pdf.
  • Chapman Printing submits a summary invoice for all jobs printed during the month. The charge for business cards will be coded to the account listed above and you will be notified of the charges. Ryder Engraving prints the name badges and the charges will be handled the same way.
